Disability, life and AD&D insurance changes

You can enroll in or change your medical, dental, vision, supplemental health, and legal benefits each fall during open enrollment or when you have a qualifying life change (such as a new family member). However, you can also change some of your optional disability, supplemental life, and accidental death and dismemberment (AD&D) coverage at any time throughout the year.

What changes can you make?

Changed your mind? You can cancel or reduce your Voluntary Disability Insurance, Supplemental Life Insurance, or AD&D Insurance at any time.

How can I change this?

You can make the changes online in UCPath. Here are the steps:

In the UCPath dashboard,[従業員アクション]>[健康と福利厚生]>[ライフ イベント/福利厚生の変更]Before you can enter a life event or benefit change, you must verify your identity by correctly answering one of the random security questions set in your profile. On the Life Events page,[AD & D、障害、または生命の福利厚生の変更]and follow the prompts that correspond to what you want to change. Your Initial Eligibility Period (PIE) determines whether you need to submit a completed Abbreviated Health Statement questionnaire.

Once you submit your benefit changes, a confirmation email will be sent to the primary email address stored in UCPath.[従業員アクション]>[個人情報]>[個人情報の概要]In can be confirmed.
